Population Health Outreach Manager

The Population Health Outreach Manager is an integral member of a multi-disciplinary team, responsible for day-to-day coordination and supervision of care teams serving individuals and families with complex medical and social needs. This role leads comprehensive assessment, service planning, health promotion, and care coordination activities with a strong emphasis on housing navigation, outreach, and training. The Manager oversees Community Health Workers, ensuring the delivery of high-quality, trauma-informed, and culturally responsive services. This position serves as a key link between care management leadership, external stakeholders, and internal teams, supporting operational excellence and client outcomes in population health and housing stability initiatives.

Ronald McDonald House New York is looking for a Family Room Associate to support our satellite centers in partnering hospitals. Social Worker, Family Support Coordinator

The Social Worker, Family Support Coordinator provides supportive services to families associated with Ronald McDonald House New York. The Social Worker, Family Support Coordinator works collaboratively with hospital partners, families, referral organizations, and supports administrative needs for the department at large.  This position reports to the Social Work Manager, and liaisons with the Family Support Team within the Operations Department.

The Family Room Associate supports the NYC Health/Hospitals Kings Family Room in Brooklyn for the Ronald McDonald House New York (RMH-NY). The Family Room Coordinator serves as a key component of the Programs team to advance the organization’s mission to serve and increase community knowledge and engagement.

Compliance Officer

The Compliance Officer (CO) is responsible for overseeing and monitoring the implementation and effectiveness of Ronald McDonald House of New York’s Compliance Program (“Program”), including developing and facilitating the delivery of training strategies. This role functions as an independent and objective role that reviews and evaluates compliance issues and concerns throughout the organization and monitors systems and processes in place to promote compliance with laws, regulations, and company policies and procedures, including the Code of Conduct. The CO shall report on the Program directly to the Chief Strategy, Partnership and Branding Officer and the Compliance Committee, with access to the Board of Directors, as appropriate. The CO will serve as an ambassador throughout RMH-NY to provide guidance to employees regarding the Company’s values and facilitate employees acting with integrity in their roles.

The position serves as a first point of contact for hospital social workers, coordinators, and families helping to facilitate referrals, reservations, and efficient intake procedures. This role plays a crucial part in maximizing occupancy and delivering high-quality guest service while maintaining the integrity and confidentiality of family data.

Community Health Worker Temp

The Community Health Worker (CHW) plays a crucial role in ensuring that patients receive comprehensive and coordinated care by addressing social determinants of health and connecting them with critical support services. CHWs work with patients, healthcare providers, and community organizations to facilitate care navigation, resource linkage, and service coordination to improve overall well-being.

Calling all AmeriCorps Service Members

Ronald McDonald House New York (RMH-NY) Public Health AmeriCorps is a workforce development program building infrastructure to advance health equity outcomes for underserved children and their families. Members perform a full year of service for RMH-NY’s targeted population across all five boroughs in NYC public and safety-net hospitals. 

Opportunity

  • Assist underserved communities by improving RMH-NY’s delivery of services 
  • Conduct local surveys, market research and more to determine Social Determinants of Health
  • Work with Family Support team to engage with patients and caregivers
